Request from Re-determinations
When requesting an appeal on multiple dates of service, all dates in question should be listed on the cover sheet. If all dates cannot be listed, “multiple” should be indicated and the applicable remittance notices should be attached to your request. This will assist us in making sure that all of your appeal requests are addressed.
